OMNI Healthcare's Center for Diabetes Care offers patients a comprehensive approach to the management of diabetes, based on an individualized patient qualified endocrinologist, a podiatrist, a nutritionist and four certified diabetes educators.

Swati Chalavarya, D.O. Swati Chalavarya, D.O. Endocrinology

Dr. Swati Chalavarya completed her medical degree from Lake Erie College of Osteopathic Medicine in Bradenton, FL and residency in internal medicine at Wellington Regional Medical Center in south Florida. She specialized in endocrinology and completed her fellowship at Larkin Hospital in Miami.

Dr. Chalavarya treats various conditions of hormone imbalances and the complications that arise from them. She created protocols to minimize patient fear of insulin, improve patient safety and integrate transgender medicine, through education and formalized each as standard practice.

Dr. Chalavarya holds medical licenses in Georgia, Nevada, Florida and Arizona. She is board-certified in Endocrinology and is proficient in English and has basic fluency in Hindi and Spanish.

Susan Glatter, APRN-BC MSN, CDE, CCD Endocrinology
Susan Glatter received her Bachelor of Science in Nursing from Molloy College - Barbara H. Hagan School of Nursing and Health Sciences and Master of Science from Adelphi University, College of Nursing & Public Health in New York. Susan is a board-certified family nurse practitioner, certified diabetes educator and clinical densitometrist. She applied her experience and knowledge in the medical management and education of patients with osteoporosis, diabetes, thyroid disease and various other endocrinology disorders to develop programs with the National Osteoporosis Foundation and the American Diabetes Association. Susan is a member of the American College of Nurse Practitioners, the International Society for Clinical Densitometry, the National Osteoporosis Foundation, the American Diabetes Association, the American Association of Diabetes Educators, and the Space Coast Clinicians.

All members of the team work closely to optimize a healthy lifestyle, and help patients avoid the potential complications of diabetes.

The American Diabetes Association Recognizes OMNI Healthcare's Centers for Diabetes Education

In November 2000, the American Diabetes Association (ADA) awarded OMNI Healthcare with the prestigious ADA Recognition Certificate for creating a quality diabetes self-management education program. Operating six centers throughout the county, OMNI Healthcare is the exclusive physician-sponsored ADA-recognized provider of outpatient diabetes education programs in Brevard.

This program allows OMNI Healthcare to empower patients by educating diabetics about their disease, providing practice skills necessary to control their blood glucose levels, and providing regular check-ups from their healthcare team. In the end, patients can successfully manage their diabetes and reduce their risk for complications.
